Doctors and Medical Science
 

Most doctors are not generally actively involved in research, they may not critically appraise research articles and their knowledge of recent advances in their field may be out of date. A vast number of research papers are published every year most of these have significant limitations even if published in the most influential journals. Much of the most generously funded research is aimed at long term commercial goals rather than for the benefit of patients.

Exercise Training in Type 2 Diabetes Mellitus
 

Ample opportunity exists within New Zealand to extend the benefits of exercise to high risk Maori and Pacific Islands people. The successful development and implementation of a structured exercise intervention is both feasible and acceptable to Polynesian New Zealanders with T2DM, and efforts are indeed required to target the diabetes epidemic in this specific ethnic population.

Making a decision about whether to move into residential care or to stay at home is a complex decision for both older people and their carers. Older adults with delirium or cognitive impairment are frequently excluded from research projects yet this is the group most likely to be confronted with this dilemma. Little is known about the best ways to provide older people and their families in this situation with support.

Cranial Melioidosis with Extradural Extension
 

This case report demonstrates an unusual presentation of melioidosis, highlighting its potential for mimicking tuberculosis both clinically as well as pathologically. It also emphasises the importance of early drainage in such cases, potentially delaying the natural history of the disease, and preventing significant morbidity even in the absence of appropriate antibiotic therapy.
